A silent gauge left uninstalled as styrene warmed and a heavy vapor rolled into five villages - how did that expose them?

The unmonitored gauge that let two thousand tonnes boil over

In this episode, we follow the sequence of failures that preceded a sudden, night-time chemical disaster on Visakhapatnam’s outskirts. We lay out how storage decisions, regulatory questions and unattended cooling systems converged and ask what people and officials might have known before the cloud moved through.

Person: Andhra Pradesh Industries Minister Mekapati Goutham Reddy Date: 7 May 2020, 2:30-3:00 AM Location: R.R. Venkatapuram village, Gopalapatnam area, Visakhapatnam outskirts Topic: 2,000 metric tonnes of styrene stored without functioning temperature sensors Event: Nationwide COVID-19 lockdown beginning March 2020 left plant idle

- The plant began in 1961, later became LG Polymers India after acquisition by LG Chem in 1997, and expanded five times between 2006 and 2018. - Under 2006 rules, Category A petrochemical expansions required federal environmental clearance; LG operated on state permits and filed for federal clearance only in 2017. - Styrene must be stored at 20-22°C; tanks were not designed for long-term storage and normally cycled material every 10-15 days. - During the COVID-19 lockdown the plant shut down, the cooling system had a computer glitch and the tanks lacked temperature sensors or alarms. - Maintenance staff arrived between 2:30 and 3:00 AM on 7 May to inspect the idle plant as a vapor cloud - heavier-than-air styrene vapour - was already forming and moving through five nearby villages.
