A former child star in Cardiff had a stereotactic anterior capsulotomy and died three weeks later; how did that expose psychosurgery?

The anterior capsulotomy that Lena Zavaroni chose and lost

In this episode, we follow the choices that led a thirty-five-year-old Lena Zavaroni to agree to a targeted brain lesion and the medical, historical, and ethical context that framed that choice. We trace the procedure she accepted, the century of practices that preceded it, and the unanswered question her death highlighted: when is it justified to destroy brain tissue to try to save a person?

Person: Lena Zavaroni Date: 1999 Location: University Hospital of Wales, Cardiff Procedure: Stereotactic anterior capsulotomy Historical origin: Gottlieb Burckhardt, December 1888

- Lena was a child star born in 1963 who developed anorexia nervosa and persistent depression from around age ten and across three decades. - By the mid-1990s she had tried inpatient care, outpatient programs, and drug regimens without lasting relief and sought neurosurgical treatment as a private patient. - The capsulotomy targeted the anterior limb of the internal capsule to disrupt circuits between the orbitofrontal cortex and thalamic nuclei and was offered at licensed centers with reported success rates of 25-70%. - Three weeks after the operation she developed pneumonia and died; no surgical error or wrongdoing was documented in the record. - Psychosurgery in the UK by 1999 was limited to two centers (Cardiff and Dundee), rooted in a lineage dating back to Burckhardt’s 1888 cortical operations.
