A rural hospital board quietly closed the obstetric unit at County General, leaving pregnant women stranded. How did that expose them?

A single hospital closed and flipped a county into a maternity desert

In this episode, we follow how decades of quiet closures, reimbursement gaps, and local budget choices removed obstetric services from hundreds of counties. We trace the data, the human calculations women make when labor starts, and the systemic factors that shifted access - and ask what it means when one decision can erase a county’s maternity infrastructure.

Person: March of Dimes Date: 2020-2022 reports Location: United States counties Topic: Maternity care deserts Statistic: 1,119 counties; 5.6 million women of reproductive age

- A county can move from low access to no access after one hospital closes its obstetric unit in a single budget decision. - Rural obstetric availability fell from 54% of rural hospitals offering OB services in 2004 to 45% in 2016, driven by gradual closures. - Medicaid’s lower reimbursement for childbirth created structural deficits that made obstetric units financially vulnerable. - March of Dimes defined a maternity care desert as a county with no hospitals offering obstetric care, no obstetric clinics, and no certified nurse-midwives. - Disparities concentrated deserts: about one in six Black infants and one in four Native American infants were born in maternity care deserts.
