Colorectal Polyps in Patients with Limited Life Expectancy

BSG From Top to Bottom

Show Notes

Dr. Jabed Ahmed and Dr. Ravi Ranjan discuss colorectal polyps in patients with limited life expectancy. The bowel cancer screening strategies rely on the long time a polyp takes before progressing to cancer by early detection and removal of polyps. Although polypectomy comes with risks of some significant complications the risk is deemed small compared to the benefit of cancer prevention. This risk benefit equation changes in patients with advanced age or multiple co-morbidities This new BSG ACP guidance developed by a multidisciplinary guidance development group aims to provide a structural framework to support clinicians and the patients in decision making in such situations. Dr. Ranjan is a Consultant physician and gastroenterologist at University Hospital of North Tees and completeing his MD at Newcastle University. He also coordinated the a BSG/ACPGBI guidance. Consultant gastroenterologists Dr Hugo Penny, Sheffield Teaching Hospitals NHS Foundation Trust and Dr. Michael Fitzpatrick, Oxford University Hospitals NHS Foundation Trust, discuss why new Coeliac Disease treatments are needed and what is on the horizon. They review the drug pipeline, which targets the known antigen and immune response: cytokine inhibitors (e.g. IL-15), gluten-degrading enzymes, tissue transglutaminase 2 (TG2) inhibitors, and tolerogenic therapies. JAK inhibitors show promise in refractory coeliac disease. With no licensed therapies and no regulatory roadmap, several agents are in phase 2 trials, with anti-TG2 therapy furthest advanced. Trials currently focus on adults with ongoing disease despite at least 12 months on a GFD, with paediatric patients a likely future focus.
