Summary
On July 17, 2007, TAM Airlines Flight 3054, an Airbus A320, tragically crashed while attempting to land at São Paulo's Congonhas Airport, resulting in the deaths of all 181 passengers and crew on board, as well as 12 individuals on the ground. The flight, which originated from Porto Alegre, faced severe weather conditions and a notoriously short runway known for its slippery surface. Despite the pilots' experience, a series of critical errors and systemic issues led to the disaster.
Captain Henrique Stefanini de Sacco and First Officer Kleber Aguilar Lima encountered significant turbulence during the flight, which compounded their challenges as they approached São Paulo. The aircraft was loaded with extra fuel, and while the pilots calculated that they could land safely despite the wet runway, they overlooked a crucial detail: one of the engines had a deactivated thrust reverser. This malfunction, combined with the pressure from the airline to maintain the flight schedule, created a dangerous situation.
As the aircraft touched down, the captain mistakenly failed to bring both thrust levers back to idle, resulting in one engine producing thrust while the other attempted to reverse. This led to the plane aquaplaning on the runway, where it ultimately crashed off the end, colliding with a cargo terminal and igniting a massive fireball. Investigations revealed that warnings about the runway conditions and the aircraft's mechanical issues had been ignored, highlighting a tragic failure in aviation safety protocols. Following the incident, significant changes were made to both the runway and the aircraft's software systems to prevent similar tragedies in the future.
