Male Bag | How to Keep Your Relationship Strong During Fertility Treatment

The Male Fertility Podcast

Show Notes

This week, we open up the Male Bag to answer a listener’s question that will resonate with anyone going through fertility treatment: “How can we stop fertility from creating distance between us?” We discuss the importance of communication, sharing our own experiences of how we learned to debrief after appointments and be vulnerable with our partners. We also discuss the power of date nights, the importance of individual time, and how to reconnect with intimacy when sex becomes a chore. 💬 Key Topics Discussed The Importance of Communication and Regular Check-ins How to Debrief After Appointments Finding Common Ground and Understanding Your Partner’s Needs The Power of Date Nights and Rediscovering Joy The Importance of Individual Time and Filling Your Own Cup Reconnecting with Intimacy and Removing the Pressure of Sex 🏁 Episode Sponsors This season is all about reminding you that when it comes to fertility, you are not alone. At The Male Fertility Podcast, we know how powerful the right support can be, which is why we’ve partnered with Fertility Family to share Impryl. We all know nutrition matters for fertility, but let’s be honest, maintaining a perfectly balanced diet for conception is incredibly hard. Choosing a supplement can be just as confusing, especially when many multivitamins contain strong antioxidants that can actually harm sperm quality. Impryl is different. It’s made using activated micronutrients that support every cell in the body, from the largest, the egg, to the smallest, the sperm. These nutrients have been clinically shown to significantly improve live birth rates in men and women who had previously been struggling to conceive. What we really like is that behind the science are real stories from real people. To find out more, visit fertilityfamily.co.uk. What happens when you're told everything is fine, but you still can't conceive? In the third and final episode of The Male Fertility Podcast’s DNA fragmentation mini-series, Tris Taylor shares his personal fertility journey, including recurrent miscarriages, unexplained infertility and discovering high sperm DNA fragmentation after years of investigations. Tris opens up about being overlooked in the fertility process, searching for answers and trying to understand what he could do to improve his reproductive health. He reflects on the lifestyle changes he made, the challenges of navigating fertility treatment and the emotional impact that stayed with him long after becoming a father. This is an honest conversation about male fertility, the importance of investigating both partners and why having a child doesn't necessarily mean the emotional impact of infertility disappears. In this episode Why Tris pushed for male fertility investigations after recurrent miscarriages. Discovering sperm DNA fragmentation and what his results revealed. Why generic lifestyle advice left him searching for clearer answers. How changes to nutrition and exercise became part of his personal approach. The isolation, shame and emotional impact of male infertility. Why the emotional effects of infertility can continue after becoming a father. Why preconception health and clear guidance matter for men.

Can you have a normal semen analysis but still experience problems with fertility? And could sperm DNA fragmentation help explain why some couples struggle to conceive, experience recurrent miscarriage or have unsuccessful fertility treatment? In this episode of The Male Fertility Podcast, we welcome consultant urologist Jonathan Ramsay for an in-depth conversation about sperm DNA fragmentation, male fertility testing and what might be happening beyond the standard semen analysis. This is the second episode in our special DNA fragmentation mini-series, in partnership with Examen Lab. Jonathan has spent decades working in male fertility and has been investigating sperm DNA fragmentation since the early development of testing. He shares his clinical experience of men who have normal or near-normal semen analysis results but whose fertility outcomes suggest there may be more to investigate. We explore the difference between single-strand and double-strand DNA breaks, why this distinction matters, and what emerging research might tell us about sperm quality and fertility outcomes. We also discuss when DNA fragmentation testing may be worth considering, including recurrent early pregnancy loss, unsuccessful ICSI cycles, poor embryo development and situations where the clinical picture doesn't quite add up. But this conversation goes beyond testing. Jonathan explains how lifestyle, nutrition, infection, varicoceles, exercise intensity and overheating may influence sperm quality. We also discuss the importance of interpreting test results in context, rather than treating one result as a definitive answer. One of the biggest themes in this conversation is the need to understand why fertility treatment fails, not simply how to improve the next outcome. Jonathan explains why better research, greater collaboration between male and female fertility specialists, and a more holistic approach to reproductive health are needed. IN THIS EPISODE, WE DISCUSS: Why a normal semen analysis doesn't necessarily mean sperm quality is normal. How ICSI changed the landscape of male fertility treatment and investigation. What sperm DNA fragmentation is and how it is measured. The difference between single-strand and double-strand DNA breaks. Why double-strand DNA damage is attracting increasing research interest. When DNA fragmentation testing may be worth discussing with a clinician. The potential relevance of DNA fragmentation in recurrent miscarriage and unsuccessful ICSI. The relationship between varicoceles, infection, lifestyle and sperm DNA quality. How exercise, cycling, overheating and extreme temperatures may affect sperm health. Why DNA fragmentation results aren't always straightforward to interpret. Whether DNA fragmentation testing could become more routine alongside semen analysis. The importance of investigating unexplained infertility and understanding why treatment fails. The future of male fertility research and the need for a more joined-up approach to fertility care.

Sperm DNA Fragmentation: What You Need to Know | Lalana Songra What is sperm DNA fragmentation, why does it matter for male fertility, and what can a DNA fragmentation test actually tell you? In this episode of The Male Fertility Podcast, we kick off a new mini-series exploring sperm DNA fragmentation, in partnership with Examen Lab. We’re joined by Lalana Songra, Chief Medical Advisor at Examen Lab, to break down the science behind sperm DNA fragmentation and explain what men and couples need to know about sperm DNA damage, testing and sperm health. We start with the basics: what DNA is, how it is packaged inside sperm, and what happens when that DNA becomes fragmented. Lalana explains the difference between single-stranded and double-stranded DNA fragmentation, why different types of damage may have different implications, and some of the factors that can contribute to sperm DNA damage. We then explore how DNA fragmentation fits into the wider fertility journey. A standard semen analysis provides important information about sperm count, motility and morphology, but it does not assess sperm DNA integrity in the same way. So when might DNA fragmentation testing be considered? Why isn't it routinely offered to every man experiencing fertility problems? And how should men understand the results they receive? We also discuss recurrent pregnancy loss, IVF, ICSI, testicular sperm retrieval, fertility clinic practice and the current evidence around DNA fragmentation testing. Importantly, this isn't a conversation about presenting DNA fragmentation as a simple answer to infertility. Lalana explains where the evidence currently stands, what we still need to understand, and why further research is needed to establish how earlier testing might influence natural conception, fertility treatment outcomes, safety and cost effectiveness. We also finish by looking at male preconception health and sperm health, including lifestyle, diet, stress, sleep and other factors that may influence sperm DNA integrity. Questions we explore in this episode What is sperm DNA fragmentation? What does DNA fragmentation actually mean? Can you have a normal semen analysis but still have sperm DNA fragmentation? What causes sperm DNA fragmentation? What is the difference between single- and double-stranded DNA fragmentation? How is sperm DNA fragmentation tested? When might DNA fragmentation testing be considered? Why isn't DNA fragmentation testing routine? Could sperm DNA fragmentation be relevant to recurrent pregnancy loss? What can men do to support better sperm health?

We're back. Briefly. This is our season finale, and before we sign off we wanted to sit down and actually tell you where we've been, what we've been up to, and what's coming. We've not been quiet. We've been at the Fertility Show, running panels, staffing a stand, talking to hundreds of people over two days. We've been building something in the community. And we've made a decision that's been a long time coming. We're moving on from the NeXYs name. The community is now the Male Fertility Hub. We sat in a meeting for an hour and a half going round the houses on what to call it, and landed back on the exact word we started with. Sometimes the obvious answer is the right one. It does what it says on the tin, and that's the point. We've also been listening. Men in the community kept asking where their partners could go for the same kind of support they were getting. We'd always assumed there was plenty out there for women, but what they meant was there was nothing specifically for the partners of men going through male factor infertility. So we built it. We now have a couples group, couples online meets, and a ladies-only group for female partners, run by Jenna and Jen. It's already making a difference. This is the last episode of the season, but there's a lot coming. We're looking forward to getting back to it. What to Expect What made this year's Fertility Show the best one yet, and the message from a colleague that stuck with us The name change: why we moved from NeXYs Fertility to the Male Fertility Hub The moment at a talk in March that finally settled the debate The new couples group, couples meets, and ladies-only group for female partners Why we built it, who runs it, and how to join What's coming in the next season and why we're more fired up than ever

You know you should be eating better, sleeping more, cutting back on alcohol, exercising differently, managing stress, and taking supplements. And knowing all of that is somehow making everything worse. In this episode, Ciaran and Shaun take on a brilliant listener question: how do you actually optimise your lifestyle without becoming obsessive about it? It is one of the most common traps men fall into during a fertility journey, and both hosts have lived it themselves. Shaun spent eight months following a strict low-GI diet to try to bring his testosterone levels up naturally after his micro-TESE, with charts inside his kitchen cupboard and every food on the glycaemic index memorised. His levels went down. Ciaran was handed a list of changes in a consultation room and immediately buried his head in the sand because the whole thing felt so overwhelming he didn't know where to start. This conversation is honest, practical and genuinely reassuring. If the pressure of getting everything right is starting to feel like another thing to fail at, this one is for you. What to Expect Why trying to do everything at once often makes things worse, not better Shaun's eight months of obsessive low-GI living and why his testosterone still went down What Ciaran's urologist said about alcohol that actually stuck Why fertility nutritionist Mel Brown told her clients to enjoy the burger Ciaran's practical framework: write down five changes, prioritise them, and introduce them one at a time How to think about consistency rather than perfection, and why one bad week doesn't matter Reframing lifestyle changes as something that outlasts your treatment, not a programme you are on The thermostat analogy that Shaun still uses years later
